About the Role
The Homeless Prevention Council is a nonprofit providing personalized case management services to support self-sufficiency and stability for residents of Lower and Outer Cape Cod. HPC is seeking a Case Manager to work with our dedicated team.
The Case Manager will:
- Assess client's needs to determine their eligibility for various programs.
- Develop individualized case plans.
- Meet with clients in the office, at their homes or at community outreach locations.
- Link individuals and families with community-based resources.
- Assist in budgeting and managing access to various types of assistance.
- Ensure confidentiality when handling all aspects of client information.
- Work with community partners to provide comprehensive case management services.
Qualifications
Experience in case management and/or a housing-related field is preferred. Proficient with Microsoft applications and database systems. Precise and confidential record keeping, excellent oral and written communication skills and a commitment to teamwork are essential. Must have a valid MA driver's license and provide own transportation. Candidates who speak languages other than English are encouraged to apply.
Compensation & Benefits:
Pay: $25.00 - $30.00 per hour
